Important Health Information for 5th grade parents
Hopefully, by now you have received the notice about the required immunization for your child to attend middle school. All rising 6th graders must have a Tdap or DTap booster BEFORE entering middle school. Once your child has this booster shot, please make sure you send a copy of the proof of immunization to the school so that it can be entered into their student record.
Thank you! Please don't hesitate to call me with any questions!
